University of Kansas Student Internship Learning Agreement

Directions: the student and the site supervisor must complete all steps.

Step 1: Before completing this form, the student and site supervisor must discuss the parameters of the internship. 

Step 2: The student and site supervisor must complete learning objectives and sign the agreement by typing all answers in this fillable pdf file. Open the file, complete all components, and save to your computer.

Step 3: The student must complete the brief informational form below and attach the learning objectives/signed agreement as directed in the last step of the form.

Step 4: Upon completion of the form, copies of the form and learning objectives/signed agreement will be emailed to:

  • Student
  • Site supervisor
  • University Career Center

Step 5: The student and site supervisor should retain signed copies of the agreement.
